Short Term and Sub Acute Care

Short Term Care new sub acute unitWhat is Sub-Acute care?

To many, this term is unknown. If brief, Sub-Acute care is a step down unit from the acute hospital setting. The patient, after completing either surgery or is stable from a complex medical procedure, is ready for discharge from the hospital. However, not well or strong enough to go home, ergo, a Sub-Acute unit, a temporary placement in a rehabilitation setting where concentrated efforts are made to bring the patient back or close to “normal”. The patient is then discharged back to the community, whether that be their home, family home, or assisted living.
